Documents Interns must obtain from the host organization/institution and submit to the International Office, after their place of internship is precise:
- Letter of Intent: The letter should be printed on official letterhead of the host organization/institution, signed, stamped and dated. The letter of intent is provided as an illustrative sample and the host organization may make required changes.
- Confirmation Letter: The confirmation letter may be obtained in addition to the letter of intent. This document may be required by certain Consulates during visa application procedures.
- Detailed Internship Program: This document needs to cover the work programme to be followed during the internship period.
- Online Application Form: The online form should be filled in after the place of internship is determined. The filled-in form should be sent to this e-mail address: erasmuspl@cu.edu.tr
Its content:
- Knowledge, skills and competence to be acquired;
- Detailed programme of the internship period;
- Tasks of the intern;
- Monitoring and evaluation plan (including by whom and how the monitoring and evaluation activities are to be performed).
The procedures to be followed after the internship procedures are initiated and the documents that needs to be submitted to the International Office:
- Up-to-date transcript;
- A written notification students that have to do internship should obtain from their own departments;
- A letter of approval from the Institutes for Post Graduate/Doctoral Students (Students may obtain a template letter from the International Office. This letter should be submitted to the student’s department and the department should communicate the letter to the relevant Institute);
- Number of a Euro bank account in Cukurova University branch of Ziraat Bank (including also the IBAN number);
- A copy of Travel-Health Insurance in English language (it may also be scanned and sent via e-mail to the following e-mail address: erasmuspl@cu.edu.tr);
- Civil Liability Insurance;
- Accidence Insurance;
- A copy of the visa stamped page of the passport, after the visa is granted;
- Invoice of the travel and visa costs for students studying at Vocational High School (in cases where the activity period of the students of the Vocational High School is until 12 months, 75% of their travel and visa costs will be covered in addition to the grant amount).
Agreements:
- Erasmus Student Placement (Internship) Mobility Agreement: It is signed between the home organization and the student, and prepared by the International Office. It includes information regarding the host organization, the duration of internship, the amount of grant, etc.
- Training Agreement and Quality Commitment: It is a document specific to each student, which covers such matters as the work programme; knowledge, skills and competencies acquired; the relation of the internship activity with the student’s diploma programme; arrangements related to qualified instructor as well as the academic recognition, etc. It is prepared in three copies by the International Office or the Faculty/Department Coordinator. and signed by the host organization, the home organization and the student.
Documents that need to be obtained from the host organization at the end of the internship period:
- Certificate of Attendance: The certificate is provided as an illustrative sample and the host organization may make required changes or use its own certificate. The certificate should be printed on official letterhead of the host organization/institution, signed and stamped.
- Student Evaluation Form: It is a form that needs to be filled in by the supervisor/mentor at the host organization. It should be sent by the supervisor/mentor to the International Office via e-mail to the following e-mail address: erasmuspl@cu.edu.tr.
Documents that needs to be filled-in and presented after returning to the home organization:
- Final Report Form: The form should be sent to erasmuspl@cu.edu.tr;
- Student Report: The report should be prepared in the form of an essay in which the intern share whatever s/he wants to share, including her/his internship experiences, the works performed, whether or not s/he has experienced any problems, etc. It needs to be sent to the following e-mail address: erasmuspl@cu.edu.tr;
- Copies of the pages of the passport with entry and exit stamps;
- A couple of photographs taken at the lace of internship and/or outside during social activities should be sent to the following e-mail address: erasmuspl@cu.edu.tr;
- A letter of recognition that the students should obtained from their departments, in cases where students have to do internship as part of their courses and they wish to have their internship recognized.
